Not enough water in mix during the pour and waited too long to do the edge.

It cracks,always.The heavy traffic areas are being moved differently than the less travelled ones,so,its brittle and cracks wherever.Might be a little bit of poor prep too on the underside.(compaction)

You keep both halves!

1 in center of doorway looks like too much weight because its broke on both sides of seam?

That is the truck door...
8"@5000psi. Reber 12" on center set on chairs.
What could we have done different?
I'm concerned the concrete is aging way to fast in a new building..

I'm no concrete guy. Just telling you what I see.
But I agree with Kindred
Looks like poor prep under concrete.
